Long-Term Care Resources for the Geriatric Pharmacist
More than one-fourth of Certified Geriatric Pharmacists work in the long-term care setting. This resource page has been designed to assist CGPs who work in LTC pharmacy with identifying and locating useful resources. A primary resource for U.S. pharmacists is membership in the American Society of Consultant Pharmacists, the organization that represents long-term care pharmacists. For Canadians, the Canadian Society of Consultant Pharmacists is the comparable organization. The Australian Association of Consultant Pharmacy is available for Australians. Australians can also check out the CCGP Australian web page.

The long-term care pharmacist will want to keep abreast of news in the long-term care field. The resources below are helpful for this purpose.
McKnights LTC News – This news source is a website that offers a complimentary daily e-mail newsletter and monthly print newsmagazine to LTC pharmacists.
Caring for the Ages – Published by AMDA, the Society for Post-Acute and Long-Term Care Medicine, this LTC newspaper is published monthly and provided free to members of AMDA. It is also available to other LTC health professionals and content can be accessed on the website. The current issue of Caring for the Ages is at this link.
LTCPharmacy.net - This website has useful resources, especially for keeping up with legislative and regulatory changes impacting long-term care pharmacy. An e-mail newsletter is also available from this website.
Patient Safety Resources

Institute for Safe Medication Practices - ISMP has a variety of useful resources related to patient and medication safety, including a newsletter for LTC nurses focused on prevention of medication errors. Check out their list of medication safety tools and resources.
National Coordinating Council for Medication Error Reporting and Prevention - has a variety of resources related to medication errors.
USP Compounding Resources - The United States Pharmacopeial Convention has developed standards on sterile compounding (USP 797) and other compounding-related standards, as well as resources on these issues.
National Healthcare Safety Network - CDC hosts this website that tracks healthcare-associated infections and provides data and resources with the goal to ultimately eliminate healthcare-associated infections. Long-term care facilities are urged to submit data to NHSN to help achieve this goal.
