The Commission for Certification in Geriatric Pharmacy (CCGP) Excellence in Geriatric Pharmacy Practice Award recognizes the Certified Geriatric Pharmacist (CGP) who fosters the principles of good pharmacy practice, demonstrates excellence in geriatric patient care, and promotes the value and rigorous standards established by CCGP.
Nominations Due August 31, 2011
Award
- Award recipient will receive a specially designed plaque
- Recipient will be recognized on a permanent plaque displayed at CCGP headquarters
- Recipient will be honored at ASCP's Annual Meeting and Exhibition and the CCGP Annual Meeting in Phoenix, Arizona
- Recipient will receive complimentary registration to the ASCP Annual Meeting and up to $1,000 travel reimbursement to attend the meeting
Qualifications of the Award Nominee
- Nominee must be a Certified Geriatric Pharmacist.
- Current CCGP Board members are not eligible for nomination.
- Nominee uses the CGP credential to advance patient care.
- Nominee made a significant contribution to the recognition of the CGP program by:
- publishing articles;
- successfully promoting the CGP designation in their own practice; and/or
- promoting the CGP program within their organization.
- Nominee served the CCGP organization through:
- serving on committees or task forces;
- making presentations on behalf of CCGP (talks, posters, etc.); and/or
- identifying and securing partners.
Nomination Process
- Candidates may be nominated by CCGP members, non-members, and consumers. Self-nominations are also encouraged.
- Nominations must be made in writing and must include the curriculum vitae of the nominee along with a narrative endorsement describing the qualifications of the candidate.
To submit a nomination electronically, send an email to info@ccgp.org. Include CCGP Excellence in Geriatric Pharmacy Award Nomination in the subject line.

Application: Attachments to the email must include:
- the nominee’s curriculum vitae
- a narrative endorsement of up to 500 words describing the nominees qualifications
- any ancillary supporting or substantiating materials, if available
- ancillary documentation that is NOT electronic, mail to CCGP headquarters
- must be received by application deadline
2011 Deadline: The deadline for receipt of nomination materials for the 2011 CCGP Excellence in Geriatric Pharmacy Practice Award is August 31, 2011, 5:00 pm eastern time. The award recipient will be announced on or about September 15, 2011. Self-nominations are encouraged.
